Job summary

An exciting opportunity has arisen for a newly qualified or experienced mental health nurse, with akeen interest in older people's mental health and dementia, to join the Oldham Memory AssessmentService on a 12-month fixed term contract. The successful candidate will use their evidence based clinicalskills to carry out complex memory assessments and support those with a suspected/ confirmeddiagnosis of dementia, their families, and carers. You will carry out initial assessments and developperson centred care plans for patients with dementia and complex needs. You will work within anexperienced and established multi-disciplinary team, delivering high quality patient care, andsupporting people to access an early diagnosis and to live well with their dementia. This includessupporting those with early onset dementia.Knowledge and skills of working with older people is preferred, as well as an understanding and/orexperience working with people living with dementia. Experience of working in a multi-disciplinaryteam and developing good working relationships with other services is required, and willingness todevelop their knowledge and skills and clinical professional development.

Main duties of the job

Undertake complex assessments both in the community and in clinical settings, using variousevidence based cognitive assessment tools.Provide specialist nursing knowledge and skills to patients referred to Oldham Memory Service.Promote and develop evidence-based practice working within NICE guidelines and national dementiastandards.Work effectively within the multi-professional team to deliver high quality care.Actively participate in various meetings and service development.Actively participate in facilitating the post diagnostic support groups and weekly dementia hub.Offering advice and information to patients and carers both in person and on the telephone.Monitoring and titration of medications initiated to treat dementia following a diagnosis.

Oldham Memory Assessment and Treatment service provide a specialist assessment, diagnosis andtreatment service for people living in Oldham concerned they might be developing dementia. Theprimary aim of the service is to provide access to an early diagnosis and promote living well withdementia, as well as providing support to the patient, their family, and carers. We provide emotionaland practical support to help people live well with dementia and offer a range of post diagnosticsupport services. We strive to provide excellent standards of dementia care and the whole team ispassionate about improving lives of those people affected by dementia.

  • First level registration. Registered Mental health nurse

Desirable

Experience

  • A broad range of clinical skills or demonstrate willingness to develop.
