Job summary
The role would involve providing specialist communication assessments to adults with a learning disability. To work alongside multi-disciplinary colleagues to develop person centred care plans for service users. This may also include facilitating training to providers, families and carers around the communication needs of an individual.
Main duties of the job
- To provide communication assessments for adults with a learning disability
- To complete and review person centred plans
- To work within a multi disciplinary team
- To facilitate training to carers
- To signpost to other services, where appropriate.
- Maintain accurate clinical documentation
