Job summary

An opportunity has arisen within the Community Mental Health Team, Old Age Psychiatry at The Meadows for a Qualified Nurse Associate to join our team.

You will be a member of a multidisciplinary team who work with service users whose lives are effected by organic and/or functional mental health problems.

We encourage personal and professional development in addition to regular supervision and support

Main duties of the job

To provide support & promote recovery to older people who are experiencing mental health/psychological distress as a result of acute physical health or social change.

To assist the nurse practitioners in assessment, planning, implementing and evaluating individual care plans.

To ensure that service users engage effectively with the agreed Plan of Care.

To offer advice and support to non-mental health staff when dealing with patients with mental health problems.

Provide regular & practical support for service users and their carers in managing dignity and independence.

Promote a positive attitude to the care of service users.

Provide support with activities of daily living.

To have the service users' needs at the fore at all times with an underpinning set of values and skills to promote independence and choice.

To maintain adequate records according to Trust Record Keeping Standards, entering appropriate details on the service users case notes as necessary.

To attend and actively participate in training sessions, team/care planning review meetings and supervision as appropriate.

To contribute to the ongoing development of the service and to actively participate in individual professional and personal development.

To undertake such other duties as may be determined from time to time within the general scope of the post and to adhere to trust policy and procedures.

  • Registered Nursing Associate on the NMC Register with a Foundation Degree
  • GCSEs Grade C or above in Maths and English Language / New GCSE Grade 4-9 in Maths and English Language OR Functional Skills Level 2 in Maths and English or Equivalent (not entry level 2)

Experience

  • Experience of working in teams under appropriate supervision as part of a multi-disciplinary team
