Job summary

Are you a passionate Occupational Therapist looking for a new and exciting challenge with older adults? An opportunity has arisen for a senior Occupational Therapist to join our community/inpatients team based in Tameside working with older people with mental health issues. Our patients come to us from many different settings needing assessment and support to remain as independent as possible. Patients can have a variety of mental and physical health conditions. The Occupational Therapy role is crucial in ensuring patients health, wellbeing and independence are appropriately assessed and facilitated. A part of the role is to support with discharge planning, working closely with the discharge social workers, including assessments on the ward and in the home setting. You will have the opportunity to work alongside the wider MDT consisting of consultant psychiatrists, nurses, CPN's, community support workers/HCA's, physiotherapists, psychologists, OT students and apprentices. This post would suit an OT who has experience of working with older adults with complex needs in a mental health setting who can provide holistic assessments and interventions.

This is a full-time post 37.5 hrs.

Main duties of the job

The core purpose of this role is to to work across the older peoples' community mental health service to embed the Occupational Therapy assessment and intervention pathway across the service within the framework of MOHO, in collaboration with the wider MDT staff.The post holder will function as part of the MDT to deliver the Interventions required to support individuals through their recovery pathway and equip them with the skills and confidence to enable a safe discharge from the service and to support assessments and decision making by care managers.The post holder will be highly skilled and competent in conducting OT assessments developing outcome based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence based outcome measuresThe post holder will provide a range of highly skilled interventions that will be delivered on both a group and individual basis and share those skills with other team members.The OT will be responsible for developing treatment plans and intervention to be delivered by the team's technical instructor.
