CARE HOME LIAISON SERVICE, BAND 6, RMN,

An exciting secondment opportunity has arisen for an experienced Band 6 mental health nurse, to join the Care Home Liaison Service. The service operates Monday to Friday 8.30am until 16.30pm. The post holder will work within a team of mental health practitioners based in Oldham offering mental health assessment, treatment and behavioural/occupational care approaches, training and advice and support within the Oldham Care Homes and intermediate care facilities. The post holder will liaise with both primary and secondary care services, the wider health and social care footprint, carers and families within their liaison role.
