Job summary

The Care Home Liaison Service provides support and advice to care homes in the borough of Rochdale, Middleton and Heywood. The service promotes holistic, person-centred care to residents of care homes who have mental health needs and are over the age of 65 or those under 65 years who have a diagnosed dementia. The focus is on maintaining residents in their current setting, providing a comprehensive assessment and contributing towards an appropriate and effective care plan. A proactive approach reduces the risk of admission to acute mental health wards.

Main duties of the job

The post holder will be based within the Care Home Liaison Team at Birch Hill Hospital. The role will support the care home staff, under the support and guidance of a qualified Registered Mental Health Nurse, in establishing care plans to support the care homes in maintaining placements. They will provide advise on how to best meet the physical, mental and emotional needs of an individual, liaising with primary and secondary care services and the wider MDT. The post holder will be required to support qualified staff in developing and delivering appropriate educational packages and advise care home staff on best practice and individual need. The role will require the post holder to have an appropriate means of transport to travel to the care homes within the borough. The expectation will be to work within Trust procedures and policies, placing individuals at the centre of their care and involving them and their families in service provision wherever possible.
