Job summary

This is an exciting time to come and work for Pennine Care NHS Foundation Trust as we have received investment from the Living Well transformation model for Older People's Services in Oldham. We are delighted to be able to offer three new clinical posts as part of the development of Older Peoples Liaison Mental Health Services. The new funding will create the following permanent full-time posts:

o Band 8b Principal Clinical Psychologist

o Band 6 Occupational Therapist

o Band 5 Assistant Psychologist

You will be part of an already established and extremely experienced team and contribute to the development of a dynamic service which will offer a range of specialist interventions to older people presenting with mental health problems. The team is closely linked with Older Adult Services in both Primary Care and Specialist services across local authority and the NHS.

We are looking for experienced, enthusiastic and caring practitioners who are passionate about improving mental health outcomes Older People. This is a great opportunity to be involved in the forefront of shaping the service and offers the successful candidates the opportunity to work in a supportive service with opportunities for development, with CPD, training and supervision.

Main duties of the job

The core purpose of this role is to to work with the Older Peoples Community Liaison Team and Orchard House Day Hospital to embed the Occupational Therapy assessment and intervention pathway across the service , in collaboration with the wider MDT staff.

The post holder will function as part of the MDT to deliver the Interventions required to support individuals through their recovery pathway and equip them with the skills and confidence to enable a safe discharge from the service and to support assessments and decision making by care managers.

The post holder will be highly skilled and competent in conducting OT assessments developing outcome based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence based outcome measures.

The post holder will provide a range of highly skilled interventions that will be delivered on both a group and individual basis and share those skills with other team members.
