Job summary

Service/Ward/Department Summary:

Bury Access Team provides an access route into mental health services. The Access Team will triage referrals to determine the most suitable pathway to meet the needs of the patient. They assess, risk manage, and provide short term support for people experiencing mental health difficulties.

Main duties of the job

Role Summary:

To work in a Single Point of Entry/Access team screening all referrals into the service.

Key Responsibilities of the Post:

To work as part of the MDT within the Access Team to provide an effective and efficient response to referrals from GP's and other Professionals.

To assess, risk manage and provide short term support for people experiencing mental health difficulties and appropriately refer on and signpost all patients referred who need assessment and/or follow up from mental health or other services.

To attend Professionals Meetings when required and to work in collaboration with other services to ensure effective communication and sharing of information

To represent mental health services within the Integrated Neighbourhood Teams and provide advice and guidance to the MDT

To participate in the overall development of the service ensuring that the principles of Clinical Governance underpin all developments.

It is expected that you will deliver care in accordance with all Pennine Care NHS Foundation Trust policies and procedures, the Mental Health Act and community care legislation.

Key Requirements of the Person Specification:

Applicants must possess RMN / RNMH / Social Worker / OT or equivalent. Completion of Preparation for Mentorship / Practice Teacher / Fieldwork Educator qualification, or willingness to complete, is essential together with significant post-registration experience.

The Team offers a comprehensive mental health needs assessment for those individuals who may require secondary care services, and is able to offer short term support in respect of any identified mental health difficulties if indicated.

  • Completion of Preparation for Mentorship / Practice Teacher / Fieldwork Educator qualification or willingness to complete
  • Evidence of on-going CPD

Desirable

  • Degree - or working towards completion.
  • Non-medical prescribing qualification
  • Formal teaching qualification.
  • Further training in brief psychosocial interventions.

Experience

Essential

  • Significant post registration experience.
  • Proven experience of working with clients with complex needs
  • Experience of working with individuals who present to mental health services in crisis and have symptoms of mental health illnes
  • Experience of working in multi-disciplinary teams.
  • Experience of providing supervision to junior staff.

Desirable

  • Primary care mental health work experience
  • Experienced in the use of outcome measures such as CORE, PHQ9 and GAD7
  • Experience of working with a psychological model of care
  • Experience of triaging referrals.

Knowledge

  • Understanding of current mental health legislation, Safe Guarding and Child Protection Legislation
  • Awareness of NICE Guidance in the treatment of mental health problems and knowledge of treatments and interventions appropriate for individuals with mental health needs
