Job summary

There are vacancy's available within our Mental Health Services for Older People Etherow Building, Tameside Hospital. Within the In-patient Unit we provide the following:

  • 14 bedded Functional Assessment Ward - Hague Suite
  • 11 bedded Organic Assessment Ward - Summer Suite

To provide a comprehensive and specialist mental health service to older people with both functional and organic illness within the inpatient service. The practitioner will also:Assist in the development and co-ordination of skills within the inpatient multi-disciplinary team.Work collaboratively across a range of disciplines to continuously promote a needs' based approach to all clients.Promote an integrated approach to service delivery.The post will encompass both day and night duty, to support the delivery of inpatient services across the 24-hour period.

Main duties of the job

To provide a comprehensive and specialist mental health service to older people with both functional and organic illness within the inpatient service. To act as the named nurse. To carry a caseload of clients and work with evidence based interventions to include:Assessment of need of clients and carersFormulation of the care plan, to include discharge planningPlanning, implementation, delivery and evaluation of programmes of nursing care in accordance with the Care Programme Approach and in collaboration with the multi-disciplinary team, referring agent, family / carer and client.Assess and monitor risk

Engaging clients in therapeutic activities/interventions to promote independence and well-beingTo keep accurate and timely professional records, ensuring confidentiality is maintainedTo liaise with all agencies, and members of the multi-disciplinary team to ensure the needs of the client are met.

  • Awareness of the holistic needs of older people with mental health problems.
  • For example the impacts of physical health change on mental health.

  • Ability to demonstrate clinical expertise in the assessment, treatment and intervention implementation and evaluation of patient care.
  • Effective communication skills
  • Positive attitude and values when working with older people

Desirable

  • Evidence of managing changes and conflict.

  • Ability to be flexible and responsive to change in clinical need.
  • To work as part of the team.
  • The post is psychologically, emotionally and physically demanding, therefore postholder need to demonstrate effective stress management.
