Job summary

The successful candidate will be able to work effectively as an Occupational Therapist have excellent communication skills, welcome a challenge and have a real desire to make a difference to the care we provide to the adult age group inclusive of evolving models of care and modernisation of mental health services.

Main duties of the job

  • To identify and assess Occupational Therapy need and plan therapeutic interventions using the Model of Human Occupation
  • To be responsible for prioritising own work load using screening tool and delegate to the Recovery and Inclusion Workers as required.
  • To work with the multidisciplinary team on the ward.
  • To liaise, plan and co-ordinate with other professional agencies, including wards, CMHT, Housing, Third sector agencies to ensure continuity of care and quality service.
  • To participate in discharge planning meetings, care plan reviews, Best Interest meetings, and informal discussions concerning patients.
  • To be responsible write accurate and concise clinical reports using Model of Human Occupation outcome measures and to ensure up to date and accurate records are maintained regarding all patient contact.
  • Comply with HCPC Occupational Therapists standards of proficiency.
