Job summary

Pennine Care NHS Foundation Trust is delighted to develop the new Community Rehabilitation Team to support service users experiencing complex psychosis. The team will support service users whoare ready to step down from inpatient rehabilitation services to support them in moving to either funded placements or their own tenancy. The aim of the team is to reduce the length of inpatientstay for service users from both acute and rehabilitation wards, incorporating least restrictive practice, the NICE guidelines for complex psychosis and the Getting It Right First Time (GIRFT) national program.

We are looking to recruit a motivated and enthusiastic Drug and Alcohol worker to join our supportive multidisciplinary team and contribute to this exciting endeavour.

Your personal development priorities will be shared and supported through our appraisal process.

We will support you in maintaining a work-life balance and can support flexible working. The team is currently based at Trust HQ in Ashton but this may be subject to change. As the team is a trust wideservice you will be expected to travel across all five boroughs within the trust footprint.

The team operates during office hours of Monday to Friday 9am-5pm

Main duties of the job

To work with a caseload of service users experiencing complex psychosis who have drug and alcohol difficulties.

To be a specialist resource for the Community Rehabilitation Team, providing expert guidance and support for the MDT regarding dual diagnosis.

To establish links with a range of services to include statutory and third sector agencies offering drug and alcohol support.

To contribute to case formulation based on the most up to date evidence based practice.
