Job summary
To provide high quality support to residential and nursing home staff, with the aim of retaining placements for service users with dementia, and other mental health needs, and avoiding unnecessary hospital admissions. The service aims to improve quality of life for nursing and care home residents.
Main duties of the job
o Assessment of residents with dementia who are presenting with Behavioural and Psychological Symptoms of Dementia (BPSD) which can challenge carers.o To be able to assess and manage residents presenting with co-morbidities of physical health and mental health needs including the impact of medication and infection.o Formulate plans of care and manage the work of support staff in delivering care which may involve modelling care techniques to care home staffo Building and developing relationships with care providers in residential and nursing home settingso Ensure interventions are evidence based and take part in audit and review of the service as requiredo To identify the need for protection in line with the Safeguarding Adults policy and comply with local procedures and attend relevant training.o Liaise with the Quality Team in ASC in relation to any quality issues with the care settings you are supporting.o Maintain confidentiality and a high standard of professionalism at all timeso Ensure all appropriate records are maintained to the required standardincluding diaries, activity sheets, travel claims, client records and databaserequirements.o To observe and practice within the legal requirements of the Mental Health Act 1983 and the Mental Capacity Act.o To oversee supervision and appraisal of junior staff.
