Job summary
To provide support, advice and signposting to the families, friends and carers of people who use the Trust's services.
To support staff to enable families, friends and carers to be recognised as key partners in care.
To promote the Triangle of Care (ToC) standards and principles, and other carer specific initiatives to services and carers.
Main duties of the job
To act as a point of contact for families, friends and carers of those who use our services by providing emotional and practical support (not for the service user).
To facilitate meaningful carer support groups which provide education andemotional support.
To undertake carers assessment of need
To apply for personal budgets for carers based on their needs
To regularly review carers needs and personal budgets
To promote and advocate carers rights
To have an increased knowledge of available carer support networks within Bury and to act as a carer resource for staff and signpost where appropriate
To collaborate effectively with a range of agencies including PCFT staff, GPs and other health and social care staff
To attend Triangle of Care Meetings
To assist and support the ongoing development and implementation of ToCprinciples and other relevant carer policies, initiatives & strategies.
To organise and actively participate in relevant meetings
To support the development, delivery and evaluation of training to carers and staff
