Job summary
To participate in the delivery of a person centred home treatment service, offering acute psychiatric care at home. This will involve implementing an agreed care plan which will include direct nursing care, monitoring of the service user's mental health state, compliance with treatment and engagement in activities of daily living. The care is delivered in the service users' home, or other environments such as residential and nursing homes.
Work is supervised from a distance as the post holder is expected to work with a level of autonomy which does not require ongoing direct supervision, however a qualified community psychiatric nurse will be available for reference and support as required.
Main duties of the job
The Community Mental Health Team is based in the Etherow Building, Tameside Hospital and provides a service for clients 65 years of age or over with mental health issues.
- To assist care co-ordinators in the implementation of prescribed care plans
- Assist patients to maintain and develop daily living skills and promote/support patients' rights, beliefs and choices taking into account issues of diversity and cultural needs.
- Assist patients/clients to maintain and improve their mobility by exercise and the use of mobility appliances
- Support patients/clients to maintain their interests, identity and emotional well-being whilst receiving various forms of care, observing and reporting any changes in their condition, finance, environment etc. to the care co-ordinator.
- To liaise with other agencies to ensure full implementation of care plans.
- To write clear concise and legible entries in service users' notes after each visit.
- Previous experience of working with older people.
