Job summary

Pennine Care is recruiting a Multi-Professional trainee Consultant/ Approved Clinician for our newly established services of Community Rehabilitation for mental health patients. Post Holder is expected to carry out the responsibilities of an Approved Clinician after the completion of their training.

Rehabilitation and High support hub (RHS) is an established directorate of Pennine Care NHS Foundation Trust which currently provides specialist rehabilitation service to nearly 150 inpatient beds. These beds are split among our Low secure units (LSU) and step down rehabilitation wards. Pennine Care is committed to maintain the continuity of rehabilitation support to patients in community. A notable development over the last 5 years is the development of a rehabilitation pathway from inpatient rehabilitation to 24 hours support accommodation. T All patients will be subject to CPA and the principal duties of the post holder will include: Senior clinical leadership to the CRT service and responsibility for the medical care of patients under the care of team.

CRT will be a fully integrated team with an expected workload of around 80 patients by end of year 3 of working. An important part of the CRT model is intensive in-reach work whilst people are on the ward.

Main duties of the job

To undertake the role of Approved Clinician for an identified client groupo Responsible for the care and treatment of mentally disordered peoplecompulsorily detained under the Mental Health Act.o Responsible for the assessment of all levels of clinical risk within an evidencebased frameworko Responsible for undertaking mental health assessments incorporating biological,psychological, cultural and social perspectiveso Responsible for leading formulation, review and treatment within the context ofthe multi-disciplinary teamo Responsible for compiling and completing statutory documentation and providingwritten AC reports under the Mental Health Acto Applies knowledge of other relevant legislation, CoP, etc. in particular, relevantparts of the Human Rights Act 1998, the Mental Capacity Act 2005 and theChildren Act.o Applies knowledge of relevant guidance issued by NICE to care and treatmentdeliveryo Undertake highly skilled assessment in determining capacity to consent totreatmento Provide highly specialist assessment of clients with severe and enduringconditions based on the appropriate use and interpretation of assessment tools

About us

Since its formation in 2002, Pennine Care NHS Foundation Trust has grown to become one of the UK's leading providers of mental health services. Pennine Care NHS Foundation Trust provides a range of mental health, learning disability and substance misuse services for General Adults living in the boroughs of Tameside, Oldham, Bury,Rochdale and Stockport.

The majority of patients in CRT will be from the Pennine Care footprint. It is expected that initially the patients will be supported to step down from rehabilitation wards into 24 hour supported accommodations or care homes. Team will support service users in achieving their maximum potential and aim for them to return to more independent living including living in their own homes.

The post Holder will be expected to liaise with the referring and community teams during the admission and discharge process. Regular liaison with the relevant acute inpatient wards, other community Mental Health Teams and Home Treatment Teams. It will be important to maintain good relations with General Practitioners and other stakeholders, eg Social Services

All referrals to the CRT will be managed through the RHS Care Pathways Team. Referral meeting are currently held on fortnightly basis. referrals meeting and allocated for assessment and report within two weeks. All patients referred to CRT will be assessed by a band 6 professional and AC input will be required in complex cases.

  • Qualified mental health professional in one of the following groups: - A first level nurse, whose field of practice is mental health or learning disabilities - A registered occupational therapist, - A registered social worker; or - A psychologist in part 14 of the register maintained by the Health and Care Professions Council
  • Educated to minimum of Master's degree level
  • Independent prescriber (V300)
  • Working with patients as covered by the Mental Health Act, Mental Capacity Act, MHA Code of Practice
  • Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs

  • Recognised teaching qualification
  • Advanced clinical practitioner Master's degree
