Orchard House Day Hospital is part of Older Peoples mental health services in Oldham.
Orchard House supports people whose mental health problems are affecting their ability to carry out their normal everyday activities and hobbies. Orchard House supports people managing the symptoms of their mental health problems and coping with the changes in life that have been caused by their mental health problems. Orchard House gives people the opportunity to learn more about mental wellbeing and explore activities and techniques which allow them to maintain their independence and their contacts in the community. Orchard House offers a range of activities and therapeutic groups which help patients to regain their confidence and manage their symptoms including wellbeing groups and anxiety management. Our aim is for patients to work towards managing their symptoms and regaining their skills and confidence in everyday activities.
This post will work directly with patients to develop skills and interests which will help maintain their recovery following discharge. The successful candidate will help to identify community resources and groups which patients can access following discharge and will introduce the patient to those resources to help make the transition.
The successful candidate will be someone who is confident to go out in the community to make links with community services and find information about activities available to access in the community.
Main duties of the job
The post holder will work as part of Orchard House Day Hospital Team, to undertake allocated duties within the team.Working alongside mental health practitioners, supporting patients, families and carers at the day hospital and also supporting patients who are on the discharge pathway from theday hospital developing interests and opportunities in the community and ensuring the highest possible standard of care in accordance with the policies and procedures of Pennine Care NHS Foundation Trust.
