Job summary
A new service is being developed allied to the adult CMHT transformation to bring a more psychological approach to treatment and care for clients with complex emotional and relational needs (CERN). This is an evidence-based approach, and we are the first older adult team within Pennine care to provide this treatment.
Main duties of the job
To use a high level of interpersonal skills, reasoning and negotiation skills toestablish a therapeutic relationship with people with complex emotional difficulties managing barriers to communication
To act as a named worker for the service users allocated to your care practising without direct supervision and with a degree of autonomy
To undertake psychological formulation and mental health and risk assessments for service users referred to the service.
To provide support and psycho education for family members and carers to promote concepts of the CERN pathway.
To participate in MDT discussions and peer/ skills sharing sessions
To provide therapeutic weekly interventions under the Structured Clinical Management pathway both 1-1 sessions and group sessions in conjunction with other practitioners
To monitor, evaluate and modify all individual and group-work interventions/treatment in order to measure progress and ensure effectiveness of intervention.
Provide advice and guidance to other members of the team
To supervise the work of junior staff where required
To be an active team member, supporting colleagues and the service with a flexible approach to duties

To liaise with and develop relationships with other agencies involved in the services care to ensure consistent multi agency approach and maintain effective communication
To make onward referrals and signpost to other services including VCSE and neighbourhood services to enable wrap around, holistic support for service users undergoing treatment to meet a broad range of need

To respond appropriately to concerns or complaints raised regarding the service or own practice according to Trust procedures and guidelines and in conjunction with team manager, using outcomes to improve service delivery
To strive to provide an excellent standard of care and positive service experience, working with experts by lived experience to shape and develop the service.
To participate in data collection, audit and research as required
To ensure all records are entered and maintained according to Trust Standards for Record Keeping and with due regard to the IG governance standards and procedures.
