Job summary
An exciting opportunity has arisen for a Registered Nurse to join the Physical Health & Treatment Team within Stockport Community Services.
Main duties of the job
The post holder will participate in the running and the ongoing development of the Depot, Clozapine & Physical Health Check Clinics for Adult Community Mental Health Services in Stockport.
The post-holder will work with both CPA and Non-CPA patients as per Pennine Care NHS Foundation Trust policy, completing relevant Care Documentation as required.
You will ensure the highest standard of nursing care provided in accordance with the policies and procedures of Pennine Care NHS Trust.
You will work across hospital and community settings, promoting continuity of care and effective communication. You will participate in the on-going development of the service and in the education programmes of nursing students and other disciplines.
You must have a clinical knowledge of relevant medications and side effects, and have a keen interest in physical health and health promotion for SMI patients.
You must be able to work with the wider Mental Health Service to provide health and care support for patients living in Stockport.
