The highly skilled Older People's Home Intervention Team in Stockport are looking for Band 6 Mental Health Practitioners to join their team and work alongside a wide variety of practitioners within the multi-disciplinary team.

The Home Intervention Team are the Gatekeeper's to our Older People's Services here in Stockport and this role will involve working closely with the CMHT, the Inpatient Mental Health Wards along with other professions.

The Older People's Home Intervention team's main aim is to support people to remain in their own home environments and avoid admissions where possible and also to support discharge in a timely, planned and safe manner.

The Older People's Home Intervention Team are expanding to a 7 day service and this is a good opportunity for you to be part of these new developments.

This is an exciting and fulfilling role and the successful candidates will need to be as enthusiastic and motivated as we already are to make this happen and continue to provide a high standard of care to our patient group and their families/carers.

There will also be excellent opportunities for continued professional development in this role which is something we encourage all our staff to do.

Main duties of the job

Key Responsibilities of the Post:

? To offer intensive home treatment as an alternative to inpatient care.? To provide a gatekeeping service for all inpatient admissions.? To facilitate early discharge/leave pathways to support discharge from inpatientservices.? To provide assessment, risk formulation/management and assessment forservice users.? To provide crisis interventions to service users.? To plan and deliver care for all service users.? To provide supervision for junior colleagues.? To actively participate in service development.? To participate in the overall development of the service ensuring that theprinciples of Clinical Governance underpin all developments.? It is expected that you will deliver care in accordance with all Pennine Care NHSFoundation Trust policies and procedures, the Mental Health Act and communitycare legislation.

  • Significant post registration experience
