Pennine Care NHS Foundation Trustare delighted to be developing a Community Rehabilitation Team across the trust footprint.
An exciting opportunity has arisen to be a part of the Multidisciplinary Team as a Social Worker .Working in a flexible /hybrid approach the post holder will provideevidenced based care and play an an integral role indelivering a recovery focused serviceas part of a whole system approach.
The multidisciplinary team will provide specialist clinical care and work within an overall framework for the delivery of community mental health services to support people with complex psychosis who are living in funded placements, including care homes and supported accommodation. Incorporating the NICE guidelines for complex psychosis and supported by the Getting It Right First Time (GIRFT) national program
Main duties of the job
Manage a caseload of service users - providing care coordination
Provide for the social care needs of service users within Community Rehabilitation Team . Act as part of a multi-disciplinary clinical team.To provide comprehensive social work support to service users and their carers / families, and involve them in all planning and actions taken, helping to develop social resilience, motivation and daily living skills.
