The post holder will undertake the role of Senior Psychological Therapist for At Risk Mental States (ARMS) and First Episode Psychosis (FEP), provide specialist consultation and supervision to ensure supervision governance standards are delivered within the "Supervision of Psychological Therapies" competency frame work (CSIP skills for health); to enable the CBT workforce to deliver NICE compliant CBT to other psychological therapists and other professionals within the Early Intervention Team.The post holder will lead the development of the Psychological Therapy Service for ARMS and Psychosis and will work collaboratively with other clinical colleagues ensuring effective delivery of psychological intervention within the Early Intervention Team.The postholder will provide specialist education and training on the substantive application of cognitive behavioural therapy for psychosis to other members of the multidisciplinary team within the Trust and other agenciesThe postholder will assess, plan, implement and evaluate programmes of highly skilled evidenced-based cognitive behavioural intervention for psychosis
Main duties of the job
Key Responsibilities of the Post:
Carry out specialist psychological assessments to identify individuals considered to be at; high risk of developing a serious mental health problem using the Comprehensive Assessment of At-Risk Mental States (CAARMS) and/or to have developed a first episode psychosis (PANSS).
To provide evidence based psychological therapies (CBTp and FI) for those with ARMS, FEP and their families/carers. Offer consultation on psychological informed approaches to working with psychosis, as well as having the lead role supporting and supervising the band 7 psychological therapists in the team.
To take a lead role in the management of referrals for psychological therapy team and to lead on the development of an increased awareness, knowledge and application of psychological interventions within the team.
The post holder will be expected to participate in supervision and continued professional development focused on evidence based interventions in psychosis. In addition you will receive individual supervision and support from the wider team.
Use of a car or access to a means of mobility to travel across the Trust is essential.
We are looking to appoint a Full-time Highly Specialist Cognitive Behavioural Therapist (CBTp), Clinical or Counselling Psychologist to work in the Early Intervention in psychosis service in Stockport.
The post holder will join a well-established and supportive early intervention in psychosis service for people aged between 14-65 who are experiencing a first episode of psychosis or at risk mental state and their families.
Clinical Be an autonomous practitioner, accepting responsibility for formulation based cognitive behavioural intervention for psychosis and take responsibility for the delivery of the clinical service on a day-to-day basis, ensuring that service user needs are met within the cognitive behavioural therapy framework. To manage a caseload and perform comprehensive assessment of service user psychological needs, plan, formulate and evaluate care delivery according to the psychological needs of service users experiencing severe mental health problems. Perform advanced clinical skills in assessment and / or diagnosis and / ortreatment of severe mental illness from a cognitive behavioural therapyframework. Advice other professionals within the Trust using specialist knowledge in the management of service users experiencing psychosis/complex needs from the cognitive behavioural therapy knowledge base. Collect, collate, evaluate and report information, maintaining accurate patient records in the management of service users receiving cognitive behavioural interventions Provide expert consultation and advice on matters related to ARMS to members of the ARMS Service, multi-disciplinary Early Intervention Service, local primary care teams, and other agencies responsible for client care. Maintain Clinical Supervision and a balance of Continued ProfessionalDevelopment to at least fulfil the BABCP requirements for continued accreditation Provide highly specialist family intervention to carers / relatives from the cognitive behavioural therapy framework, assess family needs utilising highly specialist evidence based diagnostic tools, plan family intervention through the application of highly specialist cognitive behavioural therapy principles. Work collaboratively with multi-disciplinary teams, other professionals and agencies to ensure that client needs are met, especially in relation to the management of complex needs requiring a highly specialist cognitive behavioural therapy approach. To make highly skilled evaluations and decisions about treatment options taking into account both theoretical and therapeutic models and highly complex factors concerning the development and maintenance of the psychological problem. Employing an evidenced based practice approach to therapy and has a wide understanding of empirical research findings, in which to critically evaluate treatment approaches, on an individual and team level. Provide highly specialist advice on the management of psychosis / complex needs to social services and other care agencies, in accordance with policies, procedures, guidelines and protocols. To support the multidisciplinary team in the effective substantive application of cognitive behavioural intervention to individuals and families/carers experiencing and / or coping with serious mental illness utilising a range of cognitive behavioural approaches/ psychological therapies. Ability to develop and maintain effective therapeutic alliances with clients and families, who may be challenging, highly emotive and distressed. To make highly skilled evaluations and decisions about treatment options taking into account both theoretical and therapeutic models and highly complex factors concerning the development and maintenance of the psychological problem. To be able to communicate effectively while engaged in clinical work that is highly distressing and or challenging with episodes of verbal / threat of physical aggression. Employing an evidenced based practice approach to therapy and has a wide understanding of empirical research findings, in which to critically evaluate treatment approaches, on an individual and team level. Ability to provide staff supervision, advice and consultation on the design and implementation of plans for formal psychological treatment and / or management of clients with highly complex mental health problems. Employing methods based on evidence across a wide range of care settings. Provide clinical supervision to promote best practice and facilitate the effective application of cognitive therapy which retains fidelity to the evidence base for ARMS / FEP.Education and Development Identify learning needs, develop and deliver in-house education programmes to meet the identified learning needs of the multi-disciplinary team and other agencies who care for individuals with serious mental illness. Participate in the education of pre and post registered nurses and otherprofessionals, and curriculum development where appropriate. Perform assessment of professionals enrolled on cognitive behavioural orpsychosocial intervention courses; provide supervisory structures and resources to facilitate this process. Address specific health targets e.g. RTT related to psychosis / EIS througheducation. Participate in the process of own Personal Development Planning. Participate in the personal development planning of staff in other areas requiring education in cognitive behavioural interventions. Ensure own compliance and compliance of others with regard to Trust policies and protocols mandatory training requirements. Maintain knowledge of current developments within CBT and ARMS population generally.Clinical Governance Ability to work strategically with a range of staff and stakeholders. Establish andmaintain effective professional relationships and communications with managers, mental health professionals, referring agents and those statutory and voluntary agencies which constitute the local network of mental health service delivery. Responsible for the development and implementation of policies, procedures, guidelines and patient care relevant to the delivery of cognitive behavioural therapy for psychosis and At Risk Mental States (ARMS). To assist in implementing systems to support governance, including the review of complaints and adverse clinical incidents Promote service user and public involvement activities in the specialist area, leading to service improvement. Promotes peoples equality, diversity and rights through ensuring that own and others practice is in the best interests of clients experiencing psychosis. Ensure self and members of the cognitive behavioural therapy team are aware of procedures and policies to ensure safety and prevent risk to self and others in the therapeutic and clinical environment. Ensure self and members of the therapy team are aware of procedures and policies for moving and handling, breakaway policies, risk assessment policies and adult at risk policies that are essential when assessing and managing risk. Demonstrate advanced theoretical and practical knowledge to clinical colleagues,service users and carers / relatives within the sphere of cognitive behavioural intervention / psychological therapy. Evaluate service delivery, identify areas for improvement and initiate and manage change. Manage resources effectively and efficiently; this will relate to the effective management of waiting list and times, to include decision on clinical priority and the effective use of such resources to reduce length of wait for highly specialist cognitive behavioural intervention for both the service user and there family experiencing or coping with a serious mental illness Monitor health, safety and security of self and others and promote best practice in the clinical areas. Report and investigate any adverse incidents relating to the treatment of service users with a particular emphasis on the risk associated with providing cognitive behavioural intervention making recommendations and actions taken to avoid the adverse incident in the future. Assist with Directorate annual business planning for the development and delivery of a cognitive behavioural therapy service within the secondary care environment Participate and contribute to the multi-disciplinary team meetings, team briefs etc. promoting efficient and appropriate communication across the trust. Lead in the development of a supervisory structure that will support the safe application of cognitive behavioural therapy within the clinical process, which will facilitate the development and practice of cognitive behavioural therapy. Actively involved in the supervisory and review process, including clinicalsupervision offered on an individual and group basis.Research and Development Provide education in support of change and service development related to the substantive application of cognitive behavioural therapy to the care of people with psychosis. Ability to lead clinical audit, and / or clinical research at both a local or national level within the speciality of ARMS and FEP. Identify need for, and undertake, research, clinical audit and further the clinical evidence base in order to improve quality of cognitive behavioural interventions offered to the family and individual experiencing complex mental health problems including ARMS, Psychosis and Bipolar. Disseminate research findings and audit findings through presentation toprofessional groups within the Trust and at local and national conferences.
