Service/Ward/Department Summary:

An exciting opportunity has arisen to join our well established Home Intervention Team for Older People at the Meadows.

Role Summary:

The post holder will have proven experience of working with older people presenting with both organic and functional mental health problems. You will have excellent assessment and communication skills and the ability to work flexibly according to service need.

Main duties of the job

Key Responsibilities of the Post:

  • To offer rapid and skilled assessments to individuals presenting in mental health crisis.
  • To develop care plans which will support an alternative to inpatient admission where it is safe to do so
  • To manage the work of support workers within the service
  • To work closely with carers ensuring they are supported in their role

Key Requirements of the Person Specification:

Registered Mental Health Nurse

Evidence of continuing professional development

Experience of working with older people in an inpatient and/or community setting

Ability to work within the multidisciplinary team, and with other care providers

Use of or access to a car
