The Older Peoples Community Mental Health Team in Stockport are looking for a new band 6 Practitioner to join their team and work alongside a wide variety of practitioners within the multi disciplinary team.

This role will involve working closely with the older people Home Intervention Team and the Inpatient Mental Health Wards.

Future plans may involve 7 day working. We would consider full and part time applicants.

This is a exciting and fulfilling role and the successful candidate will need to be enthusiastic and motivated to provide a high standard of care to our patient group and their families/carers.

Main duties of the job

Undertake assessment to determine the need for CMHT care and treatment.Provide appropriate cover in the absence of the Team Manager as necessary.Implementation and evaluation of care.Develop care plans for client / patients and undertake clinical activities asappropriate, ensuring a high standard of care.Ensure all appropriate client / patient and departmental records are maintained to the required standard.Work with the wider multi-disciplinary team to enhance the standard of care client / patients receive.Participate in risk assessment and formulation of risk and management plans.The administration of Depot medication according to the Drug Prescribing Policy, and administer all other medication in accordance withagreed procedures.Take a key role in communications within the multi-disciplinary team to ensure continuity of care.Promote and adhere to the Trust's values statement.Provide supervision to staff in the team according to trust policy and standards.Supervise and assess students during placements, ensuring appropriate advice and support is given.Attend team meetings and care planning meetings as required, ensuring followup action is taken.Participate in the development of staff within the team.Communicate effectively with all staff, clients / patients and carers in accordance with Trust / Directorate standards to promote high quality of care.Ensure client / patient confidentiality at all times.
