An exciting opportunity has arisen for an enthusiastic individual to take on the role of Intermediate Care Services Flow Co-Ordinator within the Operations Hub; based in the South of the county working between our Intermediate Care In-Patient Rehab Units in Cambridge and Ely.

Working as a key member of the multi-disciplinary teams (MDT) to ensure a smooth patient journey through the Intermediate Care - In-Patient Rehab service. As well as day to day admin tasks, the Patient Flow Co-Ordinator will be required to:o Gather holistic patient information pertinent to patient progress and discharge requirements.o keep accurate and succinct records in the clinical system and on the patient tracker list.o attend daily discharge planning meetings.o facilitate patient flow related meetings and follow up actions.o meet with patients and relatives to discuss discharge plans in-line with MDT recommendations.

Please note the south position will be based in Cambridge Rehabilitation Unit, Brookfields Hospital, Cambridge.

Main duties of the job

We are looking for two people with a professional attitude, excellent communication, IT and time management skills, the ability to travel between sites and would be happy to work as part of a fast-paced team, this could be the position for you.
