We are looking for an Associate Practitioner to work within the Learning Disability Partnership service, within South Cambridgeshire. The Cambridgeshire Learning Disability Partnership (LDP) consists of six integrated specialist health and social care teams as well as local authority run day and accommodation services for adults with a Learning Disability.

This is an Ideal opportunity for people who wish to develop a wide variety of skills in working with adults who have a learning disability. The successful application would be working within and MDT alongside arts therapists, nurses, occupational therapists, physiotherapists and speech and language therapists.

The role involves working with people in a variety of setting such as their own homes, supported living, day opportunities and the wider community.

Main duties of the job

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. To work with the multi-disciplinary health clinicians under their supervision.

To work with service users to provide interventions to complete specific pieces of work delegated by health practitioners.

To contribute to the regular reviews of service users as part of the multi-disciplinary health team within the LDP.

To participate within the multi-disciplinary professional team to deliver safe person centered care.

To work in partnership with families and carers to ensure that service users individual health needs are met.

To receive regular supervision from a health practitioner, for line management ie. Caseload management / annual leave / wellbeing etc.
