Job summary

We are seeking an enthusiastic Band 6 Physiotherapist to join the Integrated Stroke Early Supported Discharge and Community Neuro Rehabilitation Service.

The Integrated service provided both stroke rehabilitation and community neuro rehabilitation supporting service users with a variety of complex needs.

This is an exciting opportunity to join an established multi-disciplinary team comprising of Physiotherapists, Occupational Therapists, Speech and Language Therapists, Psychologists, Specialist Nurses, and Neuro Rehab assistants.

This role requires the successful candidate to manage a clinical caseload across the North of Cambridgeshire. Their base will be either Peterborough, Ely, Wisbech or Doddington dependent on location nearest to home.

The role involves visiting patients in their homes, wards, or community gyms. There may be times when there is a requirement to provide clinical cover elsewhere within Cambridgeshire. The role requires agile working often using virtual consultation to manage a caseload. We will provide a smart phone and laptop to support this role.

Main duties of the job

You will provide comprehensive assessments and deliver evidence-based specialist neuro Physiotherapy programmes. You will work as part of a multidisciplinary team in supporting patients with physical, cognitive, and psychosocial needs to achieve their potential.

  10. To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.
  11. Organise and manage own time, delegating work appropriately.
  12. To conduct and contribute to risk assessments and health and safety assessments and including immediately reporting any changes/newly identified risks.
  13. To be responsible for the implementation of resulting recommendations from risk and health and safety assessments.
  14. To demonstrate the importance of gaining patient and carer consent in all interventions.
  15. To effectively communicate verbally with patients, carers and colleagues using tact and persuasive skills. This may involve using skills where patients have difficulties in communication, e.g. hearing loss, diminished sight, depression, aphasia, speech problems, cognitive impairment, behavioural problems and pain.
  16. Take an active part in the development of the Service and in supporting the organisational aims and objectives of organisations models of care.
  17. To contribute to and work towards the service/organisational aims and objectives of the Integrated ESD and CNRS.
  18. To work effectively within the service and liaise appropriately with members of the MDT and other agencies by attending MDT meetings, case reviews.
  19. To ensure that accurate and timely written records are kept which comply with the Trust policy and HCPC guidance, reporting on any issues as appropriate.
  20. To provide specialist physiotherapy advice to all members of the MDT, service users and appropriate others of changes involving current care plans, patients progress and other relevant matters that pertain to the care of the patient.
  21. To support the Advanced Practitioner/band 7 with day to day planning and delivery of a high quality service.
  22. To deputise for the Advanced Practitioner/band 7 for the management of patients.
  23. Participate in annual audit plans for the Service and annual staff and patient surveys.
  24. To undertake evidence based practice to further own and teams clinical practice.
  25. To comply with the collection of data required by the service including SSNAP data.
