Job summary
We are seeking an enthusiastic Band 6 Physiotherapist to join the Integrated Stroke Early Supported Discharge and Community Neuro Rehabilitation Service.
The Integrated service provided both stroke rehabilitation and community neuro rehabilitation supporting service users with a variety of complex needs.
This is an exciting opportunity to join an established multi-disciplinary team comprising of Physiotherapists, Occupational Therapists, Speech and Language Therapists, Psychologists, Specialist Nurses, and Neuro Rehab assistants.
This role requires the successful candidate to manage a clinical caseload across the North of Cambridgeshire. Their base will be either Peterborough, Ely, Wisbech or Doddington dependent on location nearest to home.
The role involves visiting patients in their homes, wards, or community gyms. There may be times when there is a requirement to provide clinical cover elsewhere within Cambridgeshire. The role requires agile working often using virtual consultation to manage a caseload. We will provide a smart phone and laptop to support this role.
Main duties of the job
You will provide comprehensive assessments and deliver evidence-based specialist neuro Physiotherapy programmes. You will work as part of a multidisciplinary team in supporting patients with physical, cognitive, and psychosocial needs to achieve their potential.

- To contribute to and work towards the service/organisational aims and objectives of the Integrated ESD and CNRS.

- To ensure that accurate and timely written records are kept which comply with the Trust policy and HCPC guidance, reporting on any issues as appropriate.

- To support the Advanced Practitioner/band 7 with day to day planning and delivery of a high quality service.
