Job summary

An exciting opportunity has arisen for an experienced, dynamic and skilled clinician, to take on the role of a Specialist Heart Failure Nurse. The position will cover Peterborough, Huntingdonshire, East Cambridgeshire and Fenland area. You will also be expected to support the community cardiac rehabilitation team as required.

This role has been created to support the transition of a band 6 specialist heart failure nurse, into the role of a band 7.

The successful candidate will be an experienced and confident clinician with strong interpersonal, communication skill and will work autonomously.

Experience in both community and secondary care would be an advantage. The post entails providing expert supportive care for heart failure patients, preventing avoidable hospital attendances, facilitating earlier discharge from hospital and promotion of self management.

The post holder will work in collaboration with other services across the Trust and other stakeholders in order to make a positive impact on the health and well being of patients with heart failure. They will also act as a resource for patients and other health professionals.

Main duties of the job

  1. To Manage a caseload of patients with complex Heart Failure, using evidence -based practice and patient centred goals to assess, plan, implement and evaluate interventions
  2. To work as part of the multidisciplinary team with the aim of preventing admission, supporting effective discharge and providing ongoing care for patients with Chronic Heart Failure within the community
  3. To undertake a key worker role, co-ordinating multidisciplinary care as appropriate.
  4. To provide expert support, advice, and education to raise awareness of the need of the patient with Chronic Heart Failure
  5. To Undertake all aspects of clinical duties as an autonomous practitioner
  6. To support the community Cardiac Rehabilitation team in delivering Stages 1-6 of the DHs 7 Cardiac Rehabilitation Stages: manage referral and recruit patient, assess patient, develop care plan, deliver rehabilitation care (in conjunction with other providers as necessary), re-assess patient, discharge and transition to long term maintenance
  7. The post holder will be an active team participant within the service and provide in-depth clinical knowledge.

Job responsibilities

  1. Overall managerial responsibility for caseload of chronic heart failure patients within the service, acting as an expert practitioner and overseeing care carried out by other members of the HF/CR nursing team, providing advice where required.
  2. Responsible for providing expert advice on chronic heart failure issues to a wide range of people.
  3. Responsible for policy development within own specialist area.
  4. Work with the Clinical Manager to ensure planning of the workforce and future of heart failure service provision.
  5. Assess patients with Heart Failure referred from both Primary and Secondary Care providers. Discuss suitability for discharge and/or medical management with appropriate medical staff according to agreed protocols
  6. Plan, implement and evaluate individualised specialist patient care programmes based on evidence-based practice and the application of clinical reasoning skills. Formulate a self-management plan where appropriate.
  7. Ensure that standards of care and treatment are maintained, in line with National Guidance (NICE) European Guidelines, local guidelines and policies
  8. Undertake review and monitoring of patient medications, informing GP/Consultant as appropriate.
  9. To initiate clinical investigations, interpret results and take clinical decisions based on information available.
  10. Prescribe, uptitrate/down titrate patients medications in accordance to the patients clinical response to medications and reviewing the blood test results
  11. Communicate effectively and work collaboratively with multidisciplinary team to ensure a co-ordinated service. This will include service meetings, case conferences, prevention of admission and discharge planning meetings
  12. Provide advice to patients and professionals and liaise with members of the Neighbourhood Team for provision of palliative care packages
  13. Promote better understanding of cardiac conditions for patients, their families and carers, and other agencies involved with the patients through education, support and counselling.
  14. Support the Heart Failure Rehabilitation Programme, ensuring appropriate staff skill mix and cover. Make clinical decisions and liaise with MDT on appropriate patient criteria for inclusion into the programme.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.
