We are currently have an opportunity to recruit an enthusiastic and motivated experienced Band 5, (as a developmental role), or a Band 6 Physiotherapist to join the Wisbech/Fenland Neighbourhood Team. The Specialist Practitioner Physiotherapist role is based in the area surrounding Wisbech and Fenland. You would be working as part of a community based therapy team providing high quality rehabilitation to patients with physical disabilities in their home environment. This post will allow the successful candidate the opportunity to consolidate their community, clinical and managerial skills. You will be working within a team of Physiotherapists, Occupational Therapists, Assistants and alongside Nursing and Mental Health colleagues.
The ideal candidate will have a strong work ethic, sense of accountability, accompanied by an ability to work under considerable pressure at times. This post is open to professionals with the right attributes to enhance a team.

Main duties of the job
We are looking for a self-motivated individual who is able to work as an autonomous practitioner within the integrated multi professional Community Therapy team to provide a high standard of care to our patients.
Your role will include assessing, planning and developing specialist physiotherapist programmes for patients with a wide variety of complex needs. You will be responsible for your own caseload, prioritising referrals according to clinical need, signposting appropriately and undertaking assessment of patients with complex and multiple pathologies using advanced, specialist clinical reasoning skills.
Your role alsoincludessupervision of junior staff and students, overseeing patient intervention and ensuringthat Assistant Practitioners and Integrated Care Workers deliver high quality patient care.

- You will be professionally and legally accountable for all aspects for your own and delegated work and work within codes of practice and professional guidelines. You will be required to undertake holistic assessment (including those with complex presentations and multi pathologies) making use of specialist clinical reasoning skills.
- Your role includes providing specialist advice to all members of the multi-disciplinary team, service users and appropriate others, of changes involving current care plans, patients progress and other relevant matters that pertain to the care of the patient, promoting independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.

This may involve using skills where patients have difficulties in communication, e.g., hearing loss, diminished sight, depression, speech problems, cognitive impairment, behavioural problems, and pain. In doing this you will establish and maintain appropriate working relationships with patients, carers, colleagues, and other health care professionals, identifying when to refer into other services as appropriate.
- You will attend MDT Case reviews as required. This will enable you to assess patient understanding of treatment proposals, gain valid informed consent and to work within a legal framework with patients who lack capacity to consent to treatment.
