Job summary

Permanent posts are eligible for a £1500 New Hire Bonus. An initial payment of £500 will be paid in the first month and a final payment of £1000 will be paid 1 year after your start date. This will be pro-rata for part-time posts. Internal applicants are not eligible.

We have an exciting opportunity forband 5 clinical staff to join the South Cambs (Cambridge Villages and Ely) Older Adults Mental Health Neighbourhood Teams. We welcome applications from newly qualified practitioners or those looking to move on from a first qualified post to a community role.

We are a friendly and supportive MDT embedded within an exciting Integrated Care model.We provide patient focused, evidence-based mental health interventions and assessment for people aged 65 and above, with a range of functional and organic mental health illness.You will have the opportunity to manage a caseload of service users, delivering a range of clinical skills and therapeutic and recovery-based interventions.The ideal candidate will have a strong team work ethic, sense of accountability, accompanied by an ability to work under pressure at times, with support.

We support agile/hybrid working and promote regular supervision and CPD. There is an expectation for flexibility to cover all areas if required, therefore successful applicants will need to be able to travel independently and in a timely manner. Main duties of the job

  • The post holder will work collaboratively with all disciplines as part of the multidisciplinary team. The post holder will also provide high quality interventions, which will entail the development, implementation and evaluation of programmes of care for service users and provision of support to their carers. This is likely to take place in a variety of settings. The post holder will manage a caseload under the supervision of a Band 6 Mental health nurse/practitioner or above.
  • To ensure accurate record keeping using CPFT data systems.
  • To attend agreed meetings, meet supervisor and team manager on matters of service development.
  • Refer to team leader/supervisor/senior colleagues patients who may be considered to be at 'risk' or where there are safeguarding concerns.
  • To have a working knowledge and participate in activities related to the Mental Capacity Act and Care Act, undertaking training as required.
  • To develop and promote good relationships with colleagues and other agencies.
  • Liaise effectively with other members of the service team, primary healthcare teams, residential/ nursing homes, adult social care and other organisations contingent upon the client.
  • Employ specific skills to help resolve/ manage problems in older peoples' mental health.

  • To provide an ongoing assessment of the care needs, plan care in collaboration with the service user, their carers (where appropriate) and MDT colleagues. Deliver care in a safe and effective manner and ensure that needs are reviewed on a timely basis commensurate with service user needs.
  • Supporting service user medication concordance in association with MDT colleagues. This will be achieved by demonstrating a working knowledge of medications and policies commonly used in older peoples mental health services including relevant NICE guidance e.g. anti psychotic prescribing in dementia care
  • To actively engage in partnership working in respect of the patient, carers and service development
  • To have a working knowledge of current mental health and wider legislation, in addition ensuring effective application of CPA assessment and care planning principles and effective risk assessment.

  • Liaise effectively with other members of the service team, primary health care teams, residential/nursing homes, adult social care and other organisations contingent upon the client.

  • To develop and promote good relationships with service users and carers and obtaining feedback where possible.
