The community nursing team across Cambridge is currently offering opportunities for full time hours (37.5 per week) working within CPFT.
We provide a service 7 days a week, working closely with other community services and Primary Care Networks to provide high standard holistic care to the population of Cambridgeshire
You will have the opportunity to develop yourself, with extensive learning and development opportunities.
Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.
As an Integrated Care worker (Healthcare Assistant) you will work closely with the nursing team to follow a plan of care in deliver high quality, best practice care to adults with a range of complex health needs within their own homes. We are looking for caring, responsive and diverse people who share our passion to deliver our service.
We are looking for someone with:

- To undertake patients prescribed treatment/care plans, i.e., venepuncture, insulin administration and wound care, and report findings to a qualified professional in line with competency framework.
- Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy.
