Job summary

**£1500 - New Hire Bonus**

The permanent posts are eligible for a £1500 New Hire Bonus. An initial payment of £500 will be paid in the first 1 month and a final payment of £1000 will be paid 1 year after your start date. This will be pro-rata for part-time posts. Internal applicants are not eligible.

The ideal candidate will have excellent communication skills and the ability to build and maintain relationships across primary care, secondary care and 3rdsector organisations.

The Mental Health Liaison Practitioner (MHLP) will lead a small team across surgeries ensuring delivery of advice, request of service, assessment and signposting. In due course, we hope the service will be able to deliver short interventions and you will be part of developing this part of the service.

Our ideal candidate will have a strong team work ethic, accountability, accompanied by an ability to work autonomously with a desire for innovation.

Main duties of the job

The role of Mental Health Liaison Practitioner(MHLP) will be to work across the interface between Primary Care and Mental Health Services to provide support, education and problem solving approaches to the Local Primary Care Teams.

The MHLP will provide the expertise for complex referrals received, after triage by means of clinical assessment in a variety of locations including surgeries and homes visits. This will include acting as 'sign-poster' to other services.

The MHLP role is to strengthen access to provide community triage for people who may need mental health services. The post holder will ensure systems are in place to promote smooth pathways between primary care and mental health services.

They will also provide continuing education to primary care such as support and direction, GP's Health Visitors, and others.MHLP's will provide supervision for Band 6 PCMHS Practitioners
