1. Work collaboratively with the team and professional leads to support professional and practice developments including CPFT clinical supervision framework
2. Work autonomously to assess, plan, implement and evaluate programmes of care, providing specialist advice and actively seeking evidence based practice which improves the quality of care and meets the needs of patients.
3. A holistic approach to caring should be planned that will include the appropriate level of physiological; pathological; spiritual, socio-economical and cultural aspects which influence the care of clients within your specific speciality.
4. To manage a significant caseload of patients with complex diabetes, managing individual programmes of care working with patients for self management of their diabetes.
5. Provide Community Nurse Led clinics within general practice.
6. Work within the MDT Diabetes Team supporting general practice within virtual Review Clinics.
7. Work proactively to prevent admission to secondary care
8. To promote and advance non medical nurse prescribing for the benefit of people with diabetes.
9. To provide structure education programmes for people with diabetes
10. Contribute to the development of guidelines, policies and protocols for community and secondary care.
11. Develop care plans and treatment protocols for patients with diabetes in primary care.
12. Involvement in clinical audit, present and evaluate said audits.
13. Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.
14. Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security Policies.
15. The post holder must adhere to the Trust Risk Assessment and Risk Management Processes
16. The post holder must adhere to Infection Control Policies and Procedures.
17. It is a condition of your employment that you are currently registered with the NMC and it is your responsibility to maintain your professional registration.
18. The post holder must participate in clinical and other audits as required
19. Participate in clinical supervision on a regular basis
20. The post holder is required to participate in relevant emergency preparedness process for their team.
21. To be responsible for the management of and reporting on individual and team performance indicators and development of remedial action plans as required.

Person Specification

Education / Qualifications

Essential

• First level registration ENB 928 or equivalent in diabetes nursing
• ENB 998 or teaching certificate or experience in training and education
• Evidence of degree level learning
• Evidence of continuous professional development

Desirable

• Degree in health related subject
• Non medical prescriber Diabetes educator in DESMOND / PDAC or Equivalent
• Counselling qualification

Experience

Essential

• Experience in managing complex diabetes
• Experience in providing specialist education to patients, nurses and other health care professionals Ability to run own caseload in different settings.

Desirable

• Experience in audit/research/project management
• Experience of working in a community setting

Skills & Abilities

Essential

• Evidence of expert clinical skills including understanding of insulin pump therapy, structured education programmes and newer diabetes therapies
• Standard/advanced keyboard skills and ability to communicate through IT using packages such as Word, Outlook, Excel and PowerPoint . (it is best to be specific i.e. is it for data entry, analysis, formulas, creating a data base)
• Report writing skills
• Ability to work flexibly as part of a team.

• Staff management skills
• Ability to use SystmOne

• Demonstrates understanding of safeguarding issues o
• Appreciates the significance of safeguarding and interprets this accurately for all individual children and young people/vulnerable adults whatever their life circumstances.
