Job summary

There is no typical day in IST given the nature of the Service users we support:

The role will involve triaging crisis referrals with support from a senior nurse. They will work with identified service users providing assessment, identification, and treatment of the health needs of adults with a learning disability.

The role involves participating in multi-disciplinary/ multi agency working within CPFT and with Key partners either face to face or virtually. Attending team meetings and being involved in discussions about service users.

Taking on the role of Care programme Approach (CPA )co-ordinator.

The role will include appropriate information sharing with family, Carers, Police, and Probation, GP and other key stakeholders and partner agencies.

The role involves writing Positive Behaviour Support plans and offering bespoke staff training around the person as a whole.

Nurses would be involved in medication reviews and feeding back or completing joint visits with psychiatry.

The job will involve working in different settings within the community .

Main duties of the job

To participate in the delivery of specialist Learning Disability nursing service under the guidance of a senior Nurse.

With support from a senior nurse work with identified service users providing assessment, identification, and treatment of the health needs of adults with a learning disability.

To participate in multi-disciplinary/ multi agency working within CPFT and with Key partners.

The role will include appropriate information sharing with family, Carers, Police, and Probation, GP and other key stakeholders and partner agencies.

Data collection is an essential part of the role in order to provide monitoring and evaluation of the service and demonstrates compliance with key performance indicators.

To provide appropriate support and direction for tasks undertaken by support workers.

To provide supervision to junior colleagues and support workers.To provide formal and informal training to key stake holders i.e. support providers, families etc.

  1. The post holder will demonstrate knowledge and understanding of working with Adults with a Learning Disability and be willing to undertake appropriate post registration training.
  2. To have an understanding of Health and Social care policy and legislation for the service user group in relation to your professional practice.
  3. With the support of the senior nurse prioritise and maintain a personal caseload within the context of the IST.
  4. To participate in the assessment, identification, treatment and management of the specialist health and nursing needs of the learning disability population supported by IST.
  5. This includes risk assessment, assessment of complex conditions e.g. challenging behaviour including forensic issues, palliative care, mental health, epilepsy etc.
  6. To liaise and communicate effectively and sensitively with other professionals in the MDT and external agencies (e.g. Older peoples teams, Mental Health Teams, Drug and Alcohol, Multi Agency public Protection Arrangements (MAPPA) and Childrens Services) providing specialist LD advice as necessary.
  7. To provide interventions in relation to the implementation and evaluation of specialist programmes of care which support the health care of individuals or groups with Learning disabilities and complex needs
  8. To work effectively as part of a multi-disciplinary team and across partner agencies.
  9. To have a good understanding of the specific needs of the service user group including at times of crisis. e.g. during placement breakdowns, mental health and physical health relapses and periods of challenging behaviour.
  10. To have a working knowledge of the Mental Health Act (1983) and the Mental Capacity Act. To be involved in Mental Capacity Assessments and the writing of Mental Health Act Tribunal reports with support.
  11. To work collaboratively with service users, their carers and families as appropriate, to make decisions relevant to the Service Users individual needs.
