Part-time 28 hours per week
- To work as part of the multidisciplinary team with the aim of preventing admission and providing ongoing care for patients with Diabetes Disease within the community
- To manage a caseload of patients with complex Diabetes needs, using evidence based practice and patient centred goals to assess plan implement and evaluate inventions
- Promote better understanding of Diabetes for patients, their families and carers and other agencies involved with the patient through education, support and counselling.
Main duties of the job
- To work as part of the multidisciplinary team with the aim of preventing admission and providing ongoing care for patients with Diabetes Disease within the community
- To act as key worker as appropriate, co-ordinating multidisciplinary care
- To provide expert support, advice and education to raise awareness of the needs of patients with Diabetes
- To undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues
- To work closely with both primary and secondary care to ensure a safe and effective patient pathway is in place for all diabetes patients under the care of the community diabetes team
