An exciting opportunity is on offer for a motivated and enthusiastic Band 6 Mental Health Practitioner, to take on the role as a dynamic member of the Primary mental health team based in HMP/YOI Peterborough.
The successful candidate will be to work within the integrated female/male healthcare units alongside the multi-disciplinary primary healthcare team.
The post-holder will facilitate mental health assessment in a primary care setting supporting the GP about decision making in relation to secondary care referrals, support transition of service users from the care of secondary mental health services back to Primary Care and to facilitate re-access where appropriate.
This will include "signposting" to other services , undertaking short term psychological interventions with prisoners who have mild to moderate mental health difficulties. Interventions will be based on current NICE guidelines and delivered 1:1 or within group setting.
The role endeavours to provide trauma informed care, to strengthen access to mental health service by implementing the stepped care model , providing assessments for people residing in prison who may need mental health services.
Main duties of the job
The role of the Primary Care Liaison Mental Health Practitioner will be to work across the interface between Primary Care and Mental Health Services to work alongside the MHLP to facilitate mental health assessment in primary care and support GP about decision making in relation to secondary care referrals, support transition of service users from the care of secondary mental health services back to Primary Care and to facilitate re-access where appropriate.
The post holder will do this by providing support, education and problem solving approaches to the Local Primary Care Teams particularly in the area of assessment and management of mental health problems in Adults of working age.
This will include "signposting" to other services and occasionally undertake very short term interventions for clients with mild to moderate mental health difficulties.
