Community inpatient rehabilitation is the place to be! Rehabilitation in the community is at the forefront of the implementation of Discharge to Assess. You may have gained experience in the acute hospitals and now your career in community rehabilitation beckons. This role is based at Cambridge Rehabilitation Centre where you will work as part of a multidisciplinary team (MDT) to rehabilitate patients, discharged from acute hospital care enabling their community reintegration. You will be based in a purpose-built rehabilitation centre with a fully equipped therapy gym, working closely with your colleagues in other localities and across the system that provide rehabilitation and care for the population of Cambridgeshire and Peterborough. We offer excellent professional support, and you will have the opportunity to rotate to other specialist teams within CPFT such intermediate care, neuro and neighbourhood team community therapy.

Main duties of the job

Cambridge Rehabilitation Unit has a whole MDT focussed approach to rehabilitation and strives to deliver outstanding patient-focussed specialist care and therapywhich collaboratively optimises a person's potential to achieve their individual goals. You will provide assessment and occupational therapy interventions to patients in various settings from inpatient to their own homes and care homes. You will implement therapy plans as well as delegating to therapy assistants and health care assistants as indicated. You will develop your skills in sub-acute rehabilitation and discharge planning in the community working with colleagues in health, social care and the voluntary sector. You will work autonomously in the community setting travelling across the community seeing people in their own homes.
