Cambridgeshire and Peterborough NHS Foundation Trust are looking for an exceptional Band 6Tissue Viability and Continence Specialist Nurse to join our talented and committed integrated community team.
The trusts forward thinking, initiative way of working has created this dual role to improve clinical outcomes in the prevention of wounds and the management of tissue damage due to incontinence, pressure and other comorbidities as well as treating patients referred with bladder and bowel and pelvic floor dysfunction.
The role includes assessing complex patients in their home or in a nurse led clinic setting, participating in teaching and mentoring of other professionals, engaging in clinical trials. You will be required to work closely with other stakeholders.

Main duties of the job
- To work as part of the multidisciplinary team with the aim of preventing admission, supporting early discharge and providing ongoing or shared care for patients.
- To provide an advanced standard of clinical assessment evidence-based advice and education to individuals with bladder and bowel problems.
- To provide supervision to health care professionals in relation to: prevent pressure ulcers/Moisture Associated Skin Damage (MASD) and other wounds, and use of equipment.
- To undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues.
- Act as a patient advocate through the application of critical and professional knowledge and skills.
- To organise the job role effectively with regards to clinical priorities and use of time.
- To participate in evidence-based audit and research projects.

- To plan, implement and evaluate individualised specialist patient care programmes based on evidence-based practice and the application of clinical reasoning skills.
- To be professionally and legally accountable for all aspects of your own work including management of patients in your care.
- The post holder will require a high degree of communication skills in communicating highly sensitive information regarding individuals medical conditions. To ensure effective communication with the wider community and Healthcare professionals, to persuade change, influence and improve practice.
- Empathy, reassurance and negotiating skills are required to encourage patient compliance to treatment/management strategies, where there may be significant barriers to understanding in some patient groups e.g. learning disabilities, mental health.
