Job summary
The PD Pathway acts as a specialist community mental health team to patients with personality disorder. The Pathway provides specialist assessment, MBT, SCM and consultation and advice to other teams. DBT is being developed in the team.
The core element of this post will to be to coordinate all referrals into the Pathway from the acute wards, Crisis and Home treatment team and Psychiatric Liaison Service. A key target is to minimise ward admissions and provide expert clinical guidance to staff in acute psychiatric services.
The postholder will provide specialist assessment of clients referred from acute services.
In addition the postholder will have a caseload of patients within the Pathway who may be part of the MBT programme or Structured Clinical Management treatment, or, when developed DBT.
This post includes a broad range of duties such as participating in research and audit and contributing to service development.
Within the PD pathway there is an emphasis on team based, multi-disciplinary decision making. There is close liaison with other community mental health teams and acute services. Supervision is provided individually and in groups.
Main duties of the job
Liaise with other professionals to provide continuity of care across acute care and other services.
To provide specialist Mentalisation Based Treatment (MBT) and/or DBT and/or SCM to a caseload of individuals, and facilitate groups in the Personality Disorder Pathway
Provide supervision to junior members of the team
Participate in the day to day emotional work of the MDT, and overall service development and maintenance.
