Job summary
We are seeking to recruit Band 3 Health Care Assistants to our Dementia Older Adult ward in Chase Farm Hospital.
Successful applicants will work as members of the ward nursing team, supporting the Multidisciplinary team in the assessment, development, implementation and evaluation of care needs. Expectations within this role will also include cohesive working with named nurse's in facilitating delegated tasks, supporting individual care plans, supporting the management of risk with regular monitoring
Main duties of the job
- To work with the named nurse in drawing up individual care plans, carrying out risk assessments and ensuring that these are regularly monitored.
- To carry out and be responsible for specified aspects of a care plan as delegated by senior staff.
- To encourage service users, either individually or in groups, to make full use of ward facilities and therapeutic activities in line with their care plans.
- Accompany patients to various agencies, centres and other hospitals in the community and outside the Trust as required.
- Take due consideration of the cultural, social and religious beliefs of patients, relatives and visitors and respect patients as individuals whose needs include social, spiritual, emotional and physical care.
- Ensure that patients' nutritional support needs are met. This may include helping patients to get comfortable for meals, assisting with feeding where necessary and assisting with serving meals.
- Assist patients to manage their personal care needs.
- To assist trained staff to uphold a high standard of nursing care in accordance with practices and procedures.
- Assist in the maintenance of a safe and pleasant physical environment for patients and staff.
- Assist in the confidential safe keeping of patients nursing and medical records.
- Ensure the safe keeping of patient's property and valuables in accordance with policies and procedures.
- To develop and maintain appropriate relationships with patients, relatives and carers.

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.
