Consultant Psychiatrist in Medical PsychotherapyBarnet, Enfield and Haringey Mental Health NHS Trust (BEHMHT) is recruiting a part time consultant psychiatrist into Barnet Complex Emotional Needs (CEN) Pathway. This is an established post which has become vacant after the previous post holder left the Trust to pursue other interests.

Main duties of the job

The post holder will be expected:

The post-holder will provide clinical and professional leadership in the team and will take part in management activities related to the post and the wider service within which it is embedded.

Service development and quality improvement in line with the Trusts vision, values and objectives are core responsibilities.

The consultant to the CEN pathway will have clinical responsibility for patients within the MBT programme. Other patients may be seen by the post-holder for assessment, to support assessments by other team members, or as part of the consultation-liaison function of the team.

Team functioning is central to the effective working of the CEN Pathway, and the post-holder is expected to participate in regular multi-disciplinary team meetings, treatment reviews, clinical supervision of and by other members of the team, and team training. Patient reviews, including those focused on clinical risk or psychiatric medication, will be conducted jointly wherever possible, and it is not expected that the post-holder will carry an outpatient caseload. It is not required that the post-holder act as an individual or group MBT therapist but would have the opportunity to do so, as well as to deliver MBT with other team members, and to seek further training in MBT if desired.

It is desirable that the post-holder will support the delivery of Structured Clinical Management (SCM) within Core Teams, by participating in training sessions delivered by the CEN Pathway, by taking part in clinical supervision in which Core Team staff are supported with their cases, and by assessing patients from time to time at the request of the Core Team consultants who retain clinical responsibility for these patients.

The post-holder will also provide support on a consultation-liaison basis to other services within Barnet as required; this may include patients with known or suspected personality disorders who are admitted to in-patient wards within the Trust, are being seen by the Liaison Psychiatry Team, or on occasion to private providers with whom we have contracted beds. It is anticipated that the CEN Pathway will develop the function of assessing suitable patients placed out of area to support their re-integration into community services within the Borough, and the post-holder will support this development as necessary in accordance with the agreed job plan.

The post-holder is expected to ensure appropriate clinical documentation, to collaborate with GPs, hospital doctors, and primary care clinicians regarding the physical healthcare of care co-ordinated patients, and to ensure that care co-ordinated patients receive the appropriate level of physical healthcare.

The Barnet CEN Pathway, as with all teams in the Borough, is expected to actively manage their caseloads to facilitate recovery enabling patients to return to Primary Care once they are able, and to respond to new patients as they are referred.

Staff of the Barnet CEN pathway remain closely involved in the management of their clients during periods within the Acute Care Service with a view to actively facilitating discharge from in-patient, recovery house, or Crisis Resolution and Home Treatment Team care, without delay.

As a senior clinician, the post-holder has an integral role in maintaining the performance of the team, and in the further development of the service and clinical pathways, in close collaboration with the team leader and the psychology, clinical, nursing and operational leadership of the Borough.

The post-holder is expected to maintain compliance with all legal obligations as prescribed by the Mental Health Act (MHA) and the Ministry of Justice. The post-holder is expected to assess patients under the MHA and make recommendations under the MHA, and to maintain s12(2) approval for that purpose. While patients care co-ordinated by the team will not be managed under S17A Mental Health Act 1983 (Community Treatment Order), the post-holder will be part of an on-call rota for which purpose approval to act as Responsible Clinician (RC) is required.

There is no regular medical trainee attached to the team but the post-holder will actively encourage senior trainees to consider spending special interest sessions with the team, and should therefore maintain compliance with the requirements of Health Education England to act as a clinical supervisor.

The post-holder is expected to report concerns about professional conduct or competence of medical or other colleagues.

The post-holder is expected to demonstrate willingness to be flexible in altering the timetable according to service needs and to be actively involved in providing leadership and management to the service; however it is acknowledged that as this is a part time post and any such flexibility must be compatible with this. The Clinical Director for the Barnet Division will be responsible for ensuring that suitable cover arrangements are in place for the times within the job plan when the post-holder is not based within the service.
