Job summary

This is an exciting opportunity for a Consultant Psychiatrist to join the Enfield Therapy Service, a vibrant specialist psychological therapy service comprising three main activity streams: a dedicated Personality Disorder stream, a complex PTSD stream and a Psychological Therapies Stream. The post has arisen due to the current post-holder leaving the service.

Applications are welcomed from both Consultants in General Adult Psychiatry with an interest and experience in delivering psychological therapies and from Consultant Psychiatrists in Medical Psychotherapy.

Main duties of the job

The post-holder will provide psychiatric leadership and oversight of the medical care of patients under the team. They will be supported by a full-time SAS doctor, and will work closely with the ETS stream lead clinicians and Enfield Lead Psychologist to maintain a high quality of patient-care. There are also established regular meetings with senior clinicians leading similar services across the North Central London (NCL) partnership (comprising Barnet, Enfield and Haringey, and Camden and Islington Trusts) providing a supportive forum to share best practice.

About us

The Enfield Therapy Service is a multidisciplinary service (though largely comprised of psychologists) providing a range of specialist psychological therapies.

Psychologists within PTSD stream provide specialist individual and group-based interventions for people presenting with complex PTSD: phase 1 (symptom management sessions) and phase 2 (trauma-focussed therapies, including NET and EMDR). The current caseload, including waiting-list and those in active treatment in this stream is approximately 160 patients.

The PD stream is a multidisciplinary stream offering specialist interventions for people diagnosed with Cluster B personality disorders (primarily Emotionally Unstable PD), including Structured Clinical Management (SCM) and full Mentalisation Based Therapy (MBT) and Dialectical Behavioural Therapy (DBT) programmes. Staff in this stream also offer consultation, specialist supervision and training to staff in other areas of the Enfield directorate. The current caseload in this stream is approximately 280 patients.

The Psychological Therapies Stream (PTS) offers stand-alone psychological therapies, including individual CBT-based and psychodynamic therapies and group psychotherapies. The need for medical input for these cases is much less and often involves providing advice to GPs. Total number of patients is approximately 500 patients.

Psychiatric leadership of the assessment and management of patients under the care of the service, primarily within the PTSD and PD streams. Overall the post-holder might expect to have on average 2 - 3 direct patient contacts per day. The vast majority of these will be clinic-based (face to face, less frequently telephone or videolink) but occasionally home-visits may be required (less than once every 2 -3 month

Contribute psychiatric input into weekly team-based reviews of patients participating in the PD treatment programmes (MBT and DBT)

Provide consultant psychiatric input into weekly team-based discussions of patients under the care of the service. Each stream (cPTSD, PD and PTS) has its own weekly clinical meeting. Such discussions may result in advice being given to team-members working with patients, to liaison with the patients GP or other involved professionals, or in a decision to meet with the patient for psychiatric review

Occasionally patients under the care of the service require assessment under provision of the Mental Heath Act 1983 (2007) and the post-holder will be expected to participate in Mental Health Act Assessments. This may happen less than once per year. It is very rare for patients under the care of the team to be subject to CTO (there have been none in the last 5 years)

In the PD stream in particular patients, carers and other professionals involved (for example GPs, inpatient staff) will often require advice about the safe and appropriate use of any medications in relation to care, in line with NICE guidance on prescribing in personality disorder

Through the course of working with patients under the care of the team the post-holder will regularly liaise and advise other professionals involved, including CAMHS (as part of patients being transferred from CAMHS to adult services); Crisis and Home Treatment teams; Inpatient teams; the Eating Disorder service; Core Locality mental health teams and colleagues in social services

The service runs a number of Family and Carer information groups and there will be the opportunity to participate in the on-going development of this offer

The post-holder will be expected to develop their skills at psychological formulation, the assessment of mentalising capacity and assessment of appropriateness and readiness for psychotherapy. This will be supported through peer supervision within the team, regular meetings with counter-parts in neighbouring boroughs as well as opportunities to train in specific modalities of therapy (for example MBT)

The post-holder will be expected to contribute to quality-improvement initiatives, for example through participating in regular audit activity related to the business of the service

As outlined above the service is in the process of integrating with our Core Community Mental Health Teams, which will work with primary care through Primary Care Networks. The transformation is supported by additional funding to PD services and psychological therapy, and there will be changes to how services are delivered. The post holder will contribute to and clinically lead on aspects of this transformation

There will be the expectation that the post-holder will join the Trust-wide low-frequency consultant on-call rota (less than 1 in 13)

The post-holder will offer weekly supervision to the SAS doctor attached to the service.
