These are new roles created to help us deliver our plans to transform our community mental health services, as part of the NHS Long Term Plan. The transformation of our community mental health services will lead to truly integrated health and social care for adults in Barnet, Enfield and Haringey who have serious mental illness.
Our transformation plans are supported by £25m in funding and will mean the roll-out over the next three years of multi-disciplinary teams of experts working within the new Primary Care Networks and offering wraparound, holistic care to our service users and wider community. Our aim is to support the whole person as they live with, or recover from, mental illness and to offer them support with other challenges they may face, such as debt, relationship problems, housing, education or training. We will also focus resources on reaching those who currently face obstacles in getting the help they need.
Main duties of the job
MAIN TASKS AND RESPONSIBILITIES:
- Coordinate the day to day running of the Assessment and Screening pathway within the Core Team, including screening new referrals and allocating these to the Band 6 Screener and overseeing the work of the Voluntary Care Sector workers.
- Complete telephone triages/screening for service users who are referred to our service, meeting the physical, psychological, social and cultural needs of the individual and their family, using research-based knowledge and skills to maintain the individual's identity.
- Work with community organisations, charities and voluntary care sectors across Haringey to improve access to Community Mental Health Services for residents from all communities across Haringey.
- Complete over the telephone risk assessment and management plans and where necessary, arrange home visit and face to assessments.
- Complete brief assessments including Dialog+ assessments
- To be responsible and accountable for the assessment, treatment and discharge of clients who do not meet the criteria for the Core Team and to sign post them safely to relevant partner agencies and services.
- Apply specialist clinical knowledge and experience to provide education for clients to develop a greater understanding of their mental health condition and working in a client centred way with clients to help develop their awareness of their condition, life skills and coping strategies.
