Job summary
Following new investment, we are seeking to appoint a Senior Practitioner who is passionate in supporting safe and timely patient flow from Custody to Hospital, under the Mental Health Act, working in a unique system collaborative role, from referral to transfer.
The role will enhance established clinical teams at HMP Pentonville, Wormwood Scrubs and Brixton, to include system working with Magistrates and Crown Courts, Liaison & Diversion teams, Ministry of Justice, and external Acute Care and Provider Collaborative partners, for patients on remand or sentenced, who require a transfer to hospital for urgent treatment. The role will also support all Hospital Order disposals, a disposal/transfer from Custody for a patient under a Civil Section, and the Remission process, where a patient is returning to Custody, following treatment. In addition, the role will provide support and guidance in relation to complex Release & Transfers, for patients returning to a community location from Custody.
The role will form part of the London Prison Operational Model, hosted by North London Mental Health Partnership, working with provider colleagues in different Prisons, influencing delivery in accordance with National Guidance, and the London Health & Justice Improvement Programme for 2023/24 onwards.
Main duties of the job
- To provide safe and effective care pathways for all offenders identified for transfer, under s47 or s48, and Hospital Order, to external NHS or independent in-patient services, for those being remitted back to prison and to support the London-wide Service Lead, to achieve all key related strategic objectives set out in the National Guidance and equivalent
- Working alongside the responsible prison clinical teams, both In Reach and In Patient Unit, the post-holder will act as the link between external providers and the prison teams and stakeholders to ensure a coordinated approach to transfer planning, which results in effective and timely transfers in accordance with national time scales, from Date of Referral to Date of Transfer.
- Be responsible for timely referrals to external providers, working with clinical teams to support the formulation, and obtain required collateral to include documents such as MG5 and PNC, working with Liaison & Diversion colleagues and Offender Management Unit and Probation
- Throughout the duration of an offenders stay in external services the post holder will act as a point of contact to ensure the smooth flow and exchange of information and attend all relevant forums relating to the offender, to include escalation in relation to Clinical Priority, and case progression through the Courts.

- Maintain close links with the Ministry of Justice to ensure timely transfers, following confirmation of bed availability, and complete the Warrant Application
- Throughout the duration of an offenders stay in external services the post holder will act as a point of contact to ensure the smooth flow and exchange of information and attend all relevant forums relating to the offender, to include escalation in relation to Clinical Priority, and case progression through the Courts
- To support complex Release & Transfer activity for offenders, requiring a transfer to community services following release, and to support Mental Health Act Assessment process working with the designated AMHP Team, when appropriate, to support a transfer to the acute care pathway

- Work in partnership with Service Manager and Service Lead to develop mechanism that establishes effectiveness of Transfer and discharge through evidence of timely patient flow through prison, and into Hospital to receive urgent treatment under the MHA.
- Support the development of prison based care to minimise length of hospital admission and promote site based
- Develop robust evidence based protocols for the safe admission back to the prison upon discharge from
