Job summary

Our transformation plans are supported by £25m in funding and will mean the roll-out over the next three years of multi-disciplinary teams of experts working within the new Primary Care Networks and offering wraparound, holistic care to our service users and wider community. Our aim is to support the whole person as they live with, or recover from, mental illness and to offer them support with other challenges they may face, such as debt, relationship problems, housing, education or training. We will also focus resources on reaching those who currently face obstacles in getting the help they need.

The post holder will be responsible for participating and assisting in the delivery of care/services to patients, service users and carers as part of the 18-25 Transitions Team, working under the direct or indirect supervision of a registered member of staff. You will be responsible for carrying out practical tasks, offering emotional support and the monitoring of the service users mental health condition, identified as part of the service users' care plan or care pathways.

Main duties of the job

Working with colleagues in the Community Hub to support clients aged 18-25 accessing Mental Health Support.o Build a therapeutic relationship with clients using a person-centred recovery approach and provide psychologically informed interventionso Work collaboratively as part of the Core Locality Multi-Disciplinary Team and closely with appropriate intensive Pathways to support clients to develop their recovery plan and promote personal responsibility.o Support clients to access local community resources and statutory services.o To be the named key-worker for a number of clientso To facilitate group work, under the supervision of senior staff and other clinical work asrequired.o Supporting and educating Community Resources to develop mental health awareness and navigationo Act as a mentor and role model for the ability to have a meaningful life for clients.o Build relationships with the local community and statutory services and connect clients to those resources and support a 'warm handover'.o Record client clinical information on the BEH Trust's electronic patient records system (RIO).

Essential

Desirable
