2 x Band 7 Senior practitioners - Key Worker Service

We are looking for two Band 7 Key Workers - innovative system changers - who will help us realise our ambition to deliver effective support to our cohort of children, young people and their families and carers in a new and person-centred way.

You will be a reflective and compassionate professional with a thirst for learning new things. You will bring relevant experience of working with CYP who have Autism and/or a learning disability and are often seen to have behaviour that challenges.

  • You will have experience of, and a commitment to, working closely with CYP, their families, carers and networks of professionals.
  • You will be an innovative individual with a passion for understanding and meeting individual personalised needs.
  • You will be passionate about working co-productively with children, young people and their families.

This role is suitable for a wide range of skilled practitioners with experience specific to Autism, learning disabilities and children & young people.

The Key Worker is a unique and exciting role, working directly with children, young people and their families; helping them to navigate the complex support landscape; working with them courageously and intensively to avoid admission and to develop packages of support to ensure safe and stable support within the community instead. In addition, you will be playing an active part in the development of the new, national keyworking model.

Main duties of the job

1. To be a key point of contact for families/carers allocated to you, identifying gaps in their supportand enabling the system to work together to bridge those gaps

2. To work within and contribute to, detailed risk assessments for the young people; to support planning and facilitate discharge where required.

3. Facilitate the provision and implementation of tailored behavioural and emotional support to help a child or young person and their families/carers develop skills for progress and self-managementto meet their agreed goals and outcomes

4. To work as part of a team approach, maintain the highest standards of practise, competence and conduct at work, and ensure the team operates in a positive, reflective and forward thinking culture.

To ensure appropriate service representation and input to meetings, including Care Education and Treatment Reviews (CETRs) Care Programme Approach (CPA) meetings, as well as Child in Need (CIN) meetings, Child Protection Conferences, Core Groups, Looked-After Child (LAC) Reviews, and Education, Health and Care (EHC) Annual Reviews, as appropriate.
  10. To attend, contribute to, and update on cases at the local Admission Avoidance (Dynamic Support) Register meeting.
