Job summary
This is an exciting opportunity to join a network of clinicians in a newly set up service acting as a key interface between Primary and Secondary Care Mental Health Services. This interface has been initiated as part of the community mental health transformation in Haringey. The post holder will work closely with other clinicians in the Primary Care Networks to assess, formulate and offer timely, brief treatment to clients with significant mental illness, particularly those who are falling through the gap between primary and secondary care mental health services.
This is a new permanent role created to help us deliver our plans to transform our community mental health services, as part of the NHS Long Term Plan. The transformation of our community mental health services will lead to truly integrated health and social care for adults in Barnet, Enfield and Haringey who have serious mental illness.
Our transformation plans are supported by £25m in funding and will mean the roll-out over the next three years of multi-disciplinary teams of experts working within the new Primary Care Networks and offering wraparound, holistic care to our service users and wider community.
Main duties of the job
- Undertake high quality assessments of people who are identified as having a significant mental illness that may require treatment, interventions and potentially a referral to secondary mental health
- Ensuring there are no 'cliff edges' or 'gaps in services' in provision of mental health care and support, which requires an approach to working with clients based on their current mental health needs rather than
- Providing effective care, treatment and interventions to clients on a defined caseload within the Primary Care
- To facilitate timely referrals into Secondary Mental Health services, IAPT, Haringey Wellbeing Network, Haringey Safe Haven and other and other local community
- To promote the physical health of clients with mental illness by working closely with Primary Care Colleagues and the Physical Health Check Service for clients with a Severe Mental Illness.
- Provide care and treatment to clients and their carers with a service that is culturally sensitive and gives due consideration to any special needs and ensure these are of the highest standard in terms of fair access, equality of opportunity and anti-discriminatory
- Design individual programmes of care collaboratively with service users to maximize self- management and social inclusion and reflect their own goals and aspirations. Review and adjust plans according to changing service user
