Job summary

An exciting opportunity has arisen for a dynamic and experienced Band 6 nurse, with a District Nursing qualification, to join the community nursing team In Enfield.

The post offers the opportunity to manage your own caseload whilst providing leadership and supervision to a team of nurses to ensure high clinical standards of care.

You will be responsible for the holistic assessment, planning and delivery of high-quality patient care both within the patient's own homes and clinic settings.

While evidence of collaborative teamwork, good team leadership and management skills are essential, you will be fully supported by the Team manager

Essential requirements:

  • Specialist Community Practitioner (BSc Hons District Nursing)
  • RN level 1 in Adult General Nursing
  • Experience of working at a senior level.
  • Car Driver
  • NHS Experience

Main duties of the job

The caseload managers are accountable for the overall clinical and operational Care Management of an identified group of patients within a nursing team.

They ae responsible for delegating appropriate duties to qualified and unqualified members of the nursing team throughout the working day and ensuring smooth transition of patient care to the out of hours services if necessary. They will work as part of an integrated community team and takes every opportunity to develop innovative and flexible ways of working with other professionals, statutory and voluntary agencies both internal and external to the Trust, in order to ensure effective and efficient provision of health care responsive to patient's needs. The post holder should ensure that the dignity and religious and cultural beliefs of individual patients and staff members are respected.

  • RGN
  • BSc Community Nursing (District Nursing Qualification).
