Job summary

An exciting opportunity has arisen for an experienced band 6 Memory Nurse with an interest in older people's mental health to join the Haringey Memory Service based at Victoria Unit, St Ann's Hospital in Tottenham. The successful candidate will use their evidence based clinical skills to support older adults and those needing memory service assessments and their carers. You will carry out initial assessment and follow up reviews for patients with complex needs and work within an experienced and established integrated team, delivering meaningful outcomes for service users and carers.

Previous work within the care of the older people will be essential as well as having a good multidisciplinary working relationship and experience in implementing appropriate care pathways.

Excellent, communication skills are essential and should be evident in presentations. You will have the ability to work under your own initiative as well as in collaboration with colleagues to meet targets. You will also have the ability to update and/or implement new systems in line with Trust and service requirements.

As a highly motivated Band 6 nurse, you will have an excellent opportunity to develop yourself further in mentoring/supporting colleagues, contributing to key projects and performing staff appraisals.

Main duties of the job

To participate effectively as a senior member of the Haringey Memory Service and actively contribute to the development and facilitation of a patient centred approach to care: assessing, developing, implementing and evaluating the care needs of patients and their carers

To undertake complex assessments at the clinic i.e. face to face, Video link, Telephone or patients' homes using various cognitive assessment tools e.g. RUDAS, ACE111 or MMSE

To interpret the significance of clinical observations, act upon them, make decisions or/and escalate appropriately.

To provide specialist nursing knowledge and skills to patients referred to Haringey Memory Service.

To promote and develop evidence-based practice working within NICE guidelines.

To work effectively within the multi professional/ multi agency team to deliver high quality care.

To ensure that statutory and legal requirements of care are incorporated safely within each patient's programme of care.

To determine priorities and plan patient care accordingly based on assessment of risk, informing the Memory service Manager when changes in circumstances may affect standards of care.
