Job summary

The Continence Nurse Advisor contributes to the provision of continence care within the continence team. The post holder will focus attention on the assessment of continence for all patients irrespective of their dwelling. The role requires competence in specific areas of knowledge and nursing expertise and the ability to provide advice, education and support to carers, staff and patients, within the Department of Health, NMC, NICE and RCN guidelines.

The post holder will be expected to perform comprehensive specialist assessment of patients with diverse presentations and complex physical and psychological conditions and develop and deliver an individualised treatment programme.

Main duties of the job

To hold responsibility for own caseload, supervision takes the form of regular individual

sessions.

To undertake all aspects of clinical duties as an autonomous practitioner.

To undertake evidence-based audit and or research projects to further own and team's

clinical practice with support and guidance from more senior staff

Provide day to day line management and support for Band 3-4 staff and ensure the remainder of the team are appropriately supported and managed.

To be aware of and to highlight to your line manager any clinical governance issues pertaining to your role and practice.

To contribute to the delivery of health promotion activities within the service and wider.

Job description

Job responsibilities

  1. To work under the indirect supervision of the Continence Clinical Lead for complex cases and to receive support while working on strategic development of the team/service.
  2. To act as a role model and provide clinical and leadership support to Band 3-4 and appropriate delegation of tasks as required.
  3. To participate in the ongoing development of evidence-based documentation including assessment tools, clinical guidelines, policies and procedures.
  4. To work autonomously with patients and carers receiving referrals according to clinical need and priority using evidence-based practice to meet their needs.
  5. To develop and sustain relationships and networks with groups, agencies and individuals in the best interest of patients.
  6. To act as a resource of expertise and advice to staff, other health professionals and patients.
  7. To assess and implement the holistic needs of continence patients and factors that impact on the patients overall condition, raising concerns as appropriate.
  8. To provide high standards of care by assessing and clinically reasoning decisions made.
  9. To analyse and interpret data and act accordingly based on a range of options.
  10. To act as a point of contact for patients, carers and other staff members for continence advice and support

  13. To use evidence-based research to support clinical care and to audit and evaluate that care to enable improvement.

  15. To maintain adequate records within NMC requirements and work with a high level of autonomy and accountability.
  16. To Instigate and identify the need for protection of vulnerable adults, including referral to socials services, report writing and presentation to relevant bodies.
  17. To undertake or delegate health promotion and disease prevention activities with groups, individuals, and other staff.
  18. To use risk assessment tools safely to manage high risk areas and ensure compliance with Health and Safety guidance.
  19. To facilitate and participate in the delivery of continence training (internally and externally).
  20. To act as a role model to Band 3-4 staff and comply with your professional code of conduct.
  21. To participate in staff meetings, case conferences and data collection, audit, peer reviews, and surveys as required.
