Job responsibilities

JOB SUMMARY/Profile:

To work as a member of the multidisciplinary locality Adult mental health team. To provide effective care and specialist advice for people with psychosis and complex mental health needs and their carers, in order to support their wellbeing and recovery.

To liaise with other agencies in planning and assessing programmes of care. Provide formal, ongoing clinical supervision and caseload management supervision to junior and less experienced community mental health staff, as directed.

To undertake duties designed to extend clinical and managerial expertise as delegated by the manager when required. This will include deputising in the absence of the Senior Practitioner.

MAIN TASKS AND RESPONSIBILITIES:

Clinical Responsibilities

Carry overall responsibility for the management of a caseload, acting as a care co-ordinator, as defined by the Care Programme Approach (CPA).

Deliver effective care and treatment that is carried out within a multidisciplinary framework and in collaboration with other agencies.

Involve service users and their carers in planning care that includes all of their individual and diverse needs.

Design individual programmes of care collaboratively with patients to maximize self management and social inclusion and reflect their own goals and aspirations. Review and adjust plans according to changing service user needs.

Provide evidence based therapeutic interventions using developed interpersonal skills such as CBT or psycho-education.

Provide service users and their carers with clear and intelligible information which will assist them in making informed decisions about their treatment and care.

Administer medication, monitor for effects and side-effects, in accordance with NMC Standards and Trust policies.

Facilitate vocational rehabilitation and those aspects of mental health recovery concerning occupation.

Maintain clear, comprehensive and contemporaneous clinical records on all patients in response to personally generated clinical observations, in accordance with professional standards and Trust policies.

Prepare and present clinical reports as required.

Management Responsibilities

Provide formal, ongoing clinical supervision and caseload management supervision to junior and less experienced community mental health staff, as directed.

Educational Responsibilities

Undertake teaching of qualified and unqualified staff, including students.

Undertake surveys or audits as necessary to own work and participate in clinical trials/R&D activity as required.

Initiate and/or collaborate in service/clinical developments based on appropriate research evidence. Set, monitor and ensure maintenance of standards of care and clinical outcomes.

Promote health and provide specialist advice/education regarding mental heath to professionals, service users and the public in general.

Receive regular, ongoing clinical and case management supervision; attend staff development training as required. The post holder will be subject to periodic appraisal for their work performance (Individual Performance Review) and objectives will be set on an annual basis between the post holder and their manager.

Other Duties

The post holder is required to undertake the duties and responsibilities of the post with due regard to confidentiality and the Trust policies on Data Protection, Equal Opportunities, Health and Safety and Smoking.

The post holder may be required to work at alternative locations within the Mental Health Services of this Trust.

The post holder will work effectively under Service Integration with Social Care colleagues employed by the Local Authority, who will be employed under different terms and conditions of employment.

Person Specification

EDUCATION AND QUALIFICATIONS

Essential

• Registered Mental Health Practitioner.
• Dip/BSc (Hons) in Mental Health Nursing or Occupational Therapy.
• Evidence of further post-registration education and training or equivalent teaching and assessing qualification.

Desirable

• Relevant qualification in Psychosocial Interventions with people with severe and enduring mental health problems.

EXPERIENCE AND KNOWLEDGE

Essential

• Minimum two years post registration experience, one of which will be as working with people with complex mental health problems.
• Awareness of patient care within the wider context of the organisation.

Desirable

• Knowledge of the roles within the multi-professional team.

OTHER REQUIREMENTS

Essential

• Honest, trustworthy and reliable.
• An understanding and positive regard for the needs and rights of people with mental health problems and their carers.

Desirable

• Full driving licence.
• Personal experience of mental health problems.

SKILLS AND ABILITIES

Essential

• Excellent written and verbal communication skills.
• Ability to assess clinical situations quickly and use initiative.

Desirable

• Report writing skills.
• Group facilitation skills.

Essential
